This information is used only to respond to your request and communicate about scheduling.
Do not submit sensitive medical information
The website form is intended for scheduling only. Do not include health history, diagnoses, insurance information, Social Security numbers, payment information or other sensitive information.
Form processing
The website may use a third-party form delivery service to send appointment requests to the practice email inbox. Information submitted through that form may be processed by the service for delivery. You may instead call 408-642-1930 or email support@drwaltonchiro.com.
Website analytics and links
This basic website does not include advertising trackers by default. Links to phone, email and map services are operated by their respective providers and are subject to their policies.
